Need to  Know
Rashes have been seen for many years now in hepatitis C (HCV)  therapy, ever since  we started widely using ribavirin after finding that it  increased HCV cure  rates. Ribavirin can cause speckled rash all over the  body. (Peg-interferon can 
cause rash too, but it is usually just seen at  the injection sites.) It turns out that Incivek   (telaprevir), one of the HCV protease
inhibitors approved last year, is  also  associated with rash. When Incivek was presented at the U.S. Food and  Drug  Administration meetings for drug approval,this rash problem was  discussed in  great  detail....

Identification  and Management of Adverse Events
Anticipating the adverse effects of  telaprevir and boceprevir, informing patients about their risk, and being  prepared to manage them is important for safe and successful outcomes from  triple  therapy.

A real-world study of new hepatitis C protease  inhibitors in the group of  patients who have been told they should
not wait for  newer, experimental  antivirals has shown a much higher rate of serious adverse  events and treatment

Since its approval by the Food and Drug Administration in May 2011,  telaprevir, a novel  serine protease inhibitor, in combination with  peginterferon and ribavirin has been increasingly used as an effective treatment  for hepatitis C virus (HCV)[1]. In Phase II and  III clinical trials involving  over 3,800 patients, a cutaneous eruption, usually eczematous,

occurred in  56% of patients compared to 34% taking peginterferon and ribavirin alone[2].  Furthermore, 3.7% of telaprevir-treated patients developed severe cutaneous  adverse  events. These events included 11 patients considered to have
drug  reaction with  eosinophilia and systemic symptoms (DRESS), but of these, only  one was definite, two were probable, and the remainder were possible, according  to diagnostic criteria proposed by Kardaun et al[2,3]. Herein we report three  patients out of 56 patients at risk (5%)  taking telaprevir, peginterferon, and  ribavirin at our institution who developed probable  and  definite  DRESS.

Protease  inhibitors: Silver bullets for chronic hepatitis C infection?
Recent  trials evaluated the safety and efficacy of two protease  inhibitors,  boceprevir (Victrelis) and telaprevir (Incivek),
added tostandard care  with pegylated interferon and ribavirin, in patients with  chronic hepatitis  C virus (HCV) infection. These drugs open the door for  triple therapy and  other new therapies involving combinations of other  direct-acting antiviral
agents to become the new standard of care for  this  population.

This review summarizes the results  of recent phase 3 clinical trials that evaluated the safety and efficacy of  these new agents
